Patient Pre-assessment

Before you come into hospital for a procedure or operation, we will require you to complete a pre-assessment questionnaire prior to admission. This can be done via an online form or over the telephone. Our specialist nurse will ask you about your previous medical history, any medication you are currently taking and talk about your surgery with us.

Why you require pre-assessment:

The pre-assessment health check will ensure that you are in optimum health. Any potential health issues will be picked up before you come into hospital.

Our aim is to make the admission process as smooth as possible.
